Alzheimer’s and Dementia Program Data Reporting Tool (ADP-DRT) (OMB 0985-0022), and Data Collection Terms and FAQs
The Administration for Community Living’s (ACL’s) Alzheimer’s and Dementia Program (ADP), includes the Alzheimer’s Disease Supportive Service Program (ADSSP) and Alzheimer’s Disease Initiative-Specialized Supportive Services Project (ADI-SSS).

The required OMB data collection for these programs includes reporting basic demographic information, numbers and types of professionals trained, hours of direct service specific to the project, and direct service and administrative expenditures. All grantees are required to use this form to report program data.

The FAQs provide a definition of the categories of demographic information, numbers and types of professionals, direct service specific to the project, and direct service and administrative expenditures calculations.

Federal Financial Report Form (SF-425)

This report must be submitted as a “note” using an authorized GrantSolutions account.

ACL requires cumulative financial reporting through consecutive funding periods. SF425s should be signed by the certifying official in the recipient’s finance/budget office. SF425s should be submitted to ACL electronically vi GrantSolutions. The e-copy of the SF-425 should also be sent via email to the grantee’s Program Officer.

The Paperwork Reduction Act required periodic renewal of authority to collect information from ACL Alzheimer's and Dementia grant program participants. The existing OMB approval expires in June 2020. ACL is proposing minor modifications to the existing tools in an effort to provide some necessary clarifications but all collect data that better reflects the outputs of their funded programs.

The Alzheimer’s and Dementia Program Data Reporting Tool (ADP-DRT) is needed in order to:

• Collect data for performance measures used in the justification of the budget to Congress and by program, state and national decision makers.
• Effectively manage the ACL’s Alzheimer’s Disease Program at the federal, state and local levels.
• Advocate at the federal and state levels for more effective and efficient supports and services for persons with Alzheimer’s disease and their caregivers.
